Fall Creek Family Dental Membership Plan

Our dental membership plan is a membership savings plan is to help patients without insurance receive quality care in an easy and cost effective way. Members pay an annual fee to receive regular cleanings, exams and xrays along with significant discounts on other procedures.

• NO Frequency limitations
• NO Pre-authorizations
• NO Pre-existing condition limitations

This membership plan IS NOT INSURANCE. It cannot be used in conjunction with any insurance, discount plans or other specials or discounts. The membership plan has a 12 month consecutive term, effective the day you pay for the plan. Payment is due in full when you sign up for the plan. If the patient does not use the services in the 12 month term, they forfeit any remaining services and no refund will be provided. There is a one time activation fee of $89 for any patients new to our practice or current patients who have not had a cleaning with us in over 2 years.
